3. No serious traffic violations while operating a commercial motor vehicle within the last three (3) years.

4. No preventable accidents while driving a commercial motor vehicle within the last three (3) years.

5. No preventable DOT recordable accidents (collisions leading to disabling damage and immediate medical treatment away from the scene) while operating a commercial motor vehicle within the last ten (10) years.

6. No preventable accidents causing a fatality or catastrophic injury in the driving history involving a commercial motor vehicle.

7. Must be at least 21 years of age.

8. Must own a high school diploma or equivalent.
